The class-change item for Charlotte to access class 4, the Hope Sphere is exclusive to the HD remake and can be obtained by defeating the Revenant in the Woods of Wandara after talking to Grand Croix post-game. The key item must be in the party's possession to activate the High Priestess or Chaosbringer class selection in Class 4.
It has no outside effect like any other Class 4 items.
